Models of transition of care for adolescents who require Special Care Dentistry

Abstract
This paper aims to explore the literature and outline ideas for current practice concerning the transition of care for
adolescents with additional needs from paediatric dental services to adult care dental services. There is a lack of
available papers in the dental literature in the UK concerning transition and the aim of this paper is to stimulate
debate and raise awareness of the processes that can be used to facilitate seamless transition of care. Barriers to
transition are discussed and care pathways and examples of models of care are described.
