Management of a late presentation maxillary cyst in a patient with severe learning disabilities: a case report

A 45-year-old male patient with severe learning disabilities attended the Special Care Dentistry (SCD) Department
at Liverpool University Dental Hospital (LUDH) with a history of swelling to the left side of his face. It was not
possible to take a history from him due to poor compliance and limited communication skills. Physical examination
was limited and radiographic examination was not possible. Subsequent examination under general anaesthesia
revealed a large cystic lesion in the left maxilla with significant bone loss and an associated supernumerary tooth in
the anterior maxilla. The substantial size of the lesion coupled with the patient’s limited cooperation meant that the
clinical management decision was very challenging. Multiple options were considered, all of which had potential
risks and complications, but following discussion between consultants from several disciplines it was decided that
extraction of the supernumerary tooth and enucleation of the cyst with primary closure was in the patient’s best
interests.
