Clinical Success in Immediate Complete Dentures: An Alternative Approach. Editor: Michel Pompignoli, Michel Postaire and Didier Raux. ISBN 9078-2-912550-57-6. Quintessence 2008 Softback £48. 92 pages 233 mostly colour illustrations This short and beautifully illustrated text has as its focus the use of immediate complete dentures to effect the transition to edentulousness and complete denture wearing. It is the authors’ contention that in order to facilitate this transition the prosthesis should address the functional, aesthetic and psychosocial needs of the patient. That is the prosthesis should replace the lost teeth as effectively as possible, an objective that most would agree. The book begins with a short introductory chapter presenting a critique of the various techniques used to make the transition to edentulousness. These include ‘the emergency denture’ - additions of teeth to existing partial dentures; the ‘transitional prosthesis’ - a somewhat confused description of transitional partial dentures constructed specifically to re-establish stable and functional occlusal relationships and allow a period of training and adaptation to denture wearing prior to the loss of all remaining teeth (many would refer to this as a trainer denture); and the ‘complete provisional prosthesis’ - a simple, quick fix solution made before or soon after the last remaining teeth are extracted. The limitations of these approaches are well described although sweeping statements about subsequent ridge resorption and changes to maxillo-mandibular relationships do tend to jar. The chapter ends with a summary of the ‘immediate complete denture’ as defined by the authors. In essence, this is no different to the classic immediate complete denture for which posterior extractions and ridge stability is achieved before construction of the denture which is fitted when anterior teeth are extracted and any necessary bone reshaping is undertaken. In practice, such clinical indications are more and more uncommon and are limited further by the authors’ belief that success is more likely if this approach is limited to one, usually the upper, arch. The following four chapters are devoted to a detailed, illustrated clinical guide of the authors’ technique with each chapter devoted to a denture stage. This description is based largely around the treatment of one patient although there are additional illustrated references to other patient treatment to highlight particular clinical problems and alternative methods and armamentarium. The authors’ care and attention to detail is commendable and serves to remind us of what can be achieved with a careful and accurate technique. This is in essence a description of the classic immediate complete denture technique and, for the older prosthodontist, is something of a walk down memory lane with the use of twopart custom impression trays, surgical guides and bone trimming but updated using more modern materials. The final chapter usefully describes management of other clinical situations where complete immediate dentures are required but the number and/ or distribution of the remaining teeth does not follow the classic norm described by the authors. This is a compact, easily read and fully illustrated book that is narrowly and deliberately focussed on one clinical method of effecting the transition from partial to full edentulism - the complete immediate denture. This limited and narrow approach makes it difficult to recommend it for undergraduate study. Nor, I suspect, will it appeal to the general practitioner providing routine prosthodontic care though an appreciation of such a thorough and accurate clinical approach would be of value. It is, however, a text that will appeal to the specialist prosthodontist and trainee and I would recommend it to them. Dr Nick Jepson School of Dental Sciences Newcastle University
